Forms needed by agencies during the State Revolving Fund (SRF) Loan Program process are shown in approximate chronological order.

Water Treatment Facility Construction Projects

  1. SRF Initial Review Check List (Word or PDF) - Used by Public Agencies and CWP to determine the initial screening of potential projects for later SRF Loan Program funding.

  2. SRF Loan Application (Word or PDF) - Filed by Public Agency with CWP to apply for SRF Loan Program funding.

  3. SRF Loan Application Instructions (Word or PDF) - Instructions to complete and file the SRF Loan Application.

  4. Sample Draft Revenue Program Cover Letter from Public Entity (Word or PDF) - Used by Public Agencies to transmit the Draft Revenue Program with CWP.

  5. Sample Authorized Representative Resolution (Word or PDF) - A required attachment to the SRF Loan Application.

  6. Sample Local Match Resolution (Word or PDF) - Public Agency submits if requesting a 0% interest SRF loan.

  7. SRF Minority Business Enterprise/Women Business Enterprise Guidelines and associated
    forms (Word or PDF)

  8. Construction Contract Checklist (Word or PDF) - Public Agency submits with the 100% Plans and Specifications.

  9. SRF Approval to Award-ATA (Word or PDF)

  10. Plant Classification (Word or PDF)

  11. Initiation of Operation (Word or PDF)

  12. Performance Certification (Word or PDF) - Public Agency submits at end of the 1 year performance period (after construction completion)
